Lincoln Memorial Life Insurance Company

On February 1, 2023, the Guarantee Association took over policy administration services from the Special Deputy Receiver who administered policies following the liquidation of Lincoln Memorial Life Insurance Company.

To facilitate the processing of claims, please submit the following documentation to our office using the information provided below:

  1. Copy of the Death Certificate
  2. Verified Statement of Performance
  3. Itemized Funeral Home Bill for merchandise, services, and cash advance items
  4. Next of Kin/Personal Representative Release, Subrogation and Assignment Form
Note: The Next of Kin signing the form must be a blood relative of the decedent; a copy of a newspaper obituary is acceptable to show relationship. Alternatively, a Personal Representative signing the form must submit documentation establishing their right to and/or authority to execute the document.
